Requesting a High School Transcript
To expedite your request please follow these steps:
All students requesting an official transcript must complete the jcs-transcript-request-form. Once the form is completed save as “first initial of first name, last name, transcript request”. Example: jdoetranscriptrequest.
To be able to enter information and save the document you must have Adobe Reader installed on your computer. Below is the link to Adobe. Once you have saved the document as stated above you can attach it to an email. Please send the request to your appropriate JCS contact. The JCS contact information is listed below.
If you are not able to download Adobe Reader please follow these steps.
Fill out the form, print it, scan the completed document to an email and send it to your appropriate JCS Academic Counselor or the High School Registrar. Your appropriate contact is outlined below. If you are not able to scan it you may fax it or mail it.
