Field Trip signup instructions are located at the bottom of this page.


Signup Instructions

Download and fill out the following forms:

  • Registration and Medical Release Form
    This is required by law before any student may go on any field trip.PLEASE PUT THE JCS STUDENT NUMBER FOR EACH STUDENT ON THE FIELD TRIP REGISTRATION FORM - ask your EF for the number if you do not know it.
  • Field Trip Driving Form
    This is required when driving a student to or from any field trip or other event that is not your own child.

If you cannot read or open the PDF files, then you need to download Acrobat Reader - it’s free! Go here to get the newest version. After downloading you may need to set it up to be the helper file of choice in Internet Explorer.

Mail forms, along with a separate check to cover the applicable fees for each field trip to:
